- Case Information Cover Sheet: This one-page form identifies the type of action and includes information regarding the parties and children.
- Petition for Dissolution of Marriage (Joint): FL Divorce 201. This nine-page form identifies the parties and their children and asks the court to end the marriage on the grounds, for example that it is irretrievably broken. It sets for the terms and conditions of property distribution and a proposed parenting plan. The respondent may join the action, which means that he or she agrees to the terms and conditions set forth in the petition.
- UCCJEA Information Sheet: This declaration certifies that the parties comply with the UCCJEA and establishes the place of residence of minor children involved in the action.
- Confidential Information Form: FL All Family 001. This form profiles the particulars of divorce and keeps certain information out of public access.
- Addendum to Confidential Information Form: FL All Family 002. This form provides additional information about the case.
- Financial Declaration (Petitioner): FL All Family 131. A six-page form, completed by the petitioner, declares his or her income and expenses on a monthly basis, and is used to disclose the information to the respondent and the court.

- Marital Settlement Agreement: This notarized agreement entered into between a husband and a wife establishing the terms and conditions of their divorce and resolving such matters as property division, debts, child custody, child support, visitation, debt, alimony, insurance, tax, and settlement of any property issues when they end their marriage to each other. Signed by both parties, the MSA is approved by a judge.
- Note for Dissolution Calendar (Non-Contested Case- Option Use): This form can be used to schedule a hearing in dissolution. Its use is optional in uncontested actions.
- Final Parenting Plan: FL All Family 140. This is the final plan that must be approved by the court.
- Findings of Fact and Conclusions of Law: FL Divorce 231. The finds are a review of the action from the filing in preparation for the Decree.
- Decree of Dissolution of Marriage: FL Divorce 241. This is the court order that ends the marriage. The terms set forth in the Marital Settlement Agreement are incorporated by reference.
- Order of Child Support: The order establishes appropriate child support paid by the noncustodial parent.
