Understanding the Maine Divorce Papers:
- Confidential Family Matter Summary Sheet: The Confidential Family Matter Sheet, a one-page form, identifies the parties and the type of action. Used for reference by the Clerks and the Judge.
- Complaint for Divorce (With Children): FM-004. A two-page form, the complaint identifies the parties and their children, establishes the court's jurisdiction, states the grounds, and notes real estate interests of the parties. It also establishes the five-year residential history of the children, and whether any other party has physical custody or claims visitation rights to the children.
- Affidavit Concerning Child Custody: This Affidavit Regarding Minor Children is required for all legal decision making (custody) cases. The document proves the information to establish court jurisdiction of the children.
- UCCJEA Information Sheet: This affidavit certifies that the parties comply with the UCCJEA and establishes the place of residence of minor children involved in the action.
- Child Support Affidavit (Plaintiff): FM-050. The Child Support Affidavit stipulates the terms and conditions of child support as they apply to the plaintiff.
- Child Support Affidavit (Defendant): FM-050. The Child Support Affidavit stipulates the terms and conditions of child support as they apply to the defendant.

- Entry of Appearance: FM-020. The Entry of Appearance, filed by the Defendant, is used when he or she wishes to testify to all of some of the following issues: alimony, child support, attorney's fees, and the division of marital and non-marital property.
- Marital Settlement Agreement: This notarized agreement entered into between a husband and a wife establishes the terms and conditions of their divorce and resolving such matters as property division, debts, child custody, child support, visitation, debt, alimony, insurance, tax, and settlement of any property issues when they end their marriage to each other.
- Schedule for Visitation of Minor Children: This schedule shows the visitation and parenting time routine for the noncustodial parent.
- Child Support Schedule: The schedule reflects what both parents must pay in child support.
- Child Support Worksheet: FM-040. The Child Support Worksheet is used to make the calculations by which child support is determined.
- Supplemental Child Support Worksheet: Supplemental Child Support Worksheet is used when parents provide substantially equal care. FM-040-S must be prepared first.
- Judgment of Divorce: This court order ends the marriage.
