- Personal Data Sheet: Form (NHJB-2077-FS) is a cover sheet, identifies the parties, their addresses, the children, and the type of action. The data sheet is often used as a reference by the clerks and judge and is placed in the front of the case file.
- Joint Petition for Divorce: The Joint Petition (form NHJB-2058-FS) can be filed when both spouses agree to the divorce, and it saves the cost of formal service of process. This two-page form, which must be notarized and signed by both parties, normally lists "[i]rreconcilable differences [that] have caused the irremediable breakdown of the marriage or civil union" as the grounds.
- Permanent Agreement: Called the marital settlement agreement in other jurisdictions, the permanent agreement memorializes the terms and conditions of their divorce and resolving such matters as property division, debts, child custody, child support, visitation, debt, alimony, insurance, tax, and settlement of any property issues when they end their marriage to each other. Signed by both parties, the Permanent Agreement is approved by a judge.
- Schedule for Visitation/Parenting Time of Minor Children: This form spells out the custody regime and establishes the visitation routine for the custodial and noncustodial parent.
- Appearance and Waiver of Service: This form is filed by the respondent and acknowledges his or her participation in the divorce and waives proper service of the Joint Petition for Divorce.

- Uniform Support Order: Form NHJB-2066-FS, the Uniform Support Order, stipulates the alimony and/or child support paid by the Obligor spouse as part of the terms and conditions of the divorce. It is also details the medical support obligation of each parent. The Uniform Support Orders applies in conjunction with New Hampshire Standing Orders in Domestic Cases.
- Declaration Under the Uniform Child Custody Jurisdiction Act: This declaration certifies that the parties comply with the UCCJEA and establishes the place of residence of minor children involved in the action.
- Notice of Hearing: If a hearing is needed, this documents provides the date , time and location of the hearing to the respondent.
