- Appearance: The Appearance identifies the parties in the divorce and their children. Filed in all dissolutions, this form, which the petitioner files, tells the court whether the action is pro se or not.
- Verified Petition for Dissolution of Marriage: Form PS-31152-14. The Petition establishes the history of the marriage, asks for the desired relief and asks the court to end the marriage. The petition may ask for temporary custody and child support, temporary visitation and a temporary division of assets and liability and temporary alimony. Provisional Orders prohibit the parties from dissipating assets and improperly removing children from the state.
- Summons: Form TCM-TR-4.1-2. The Summons informs the respondent that the petitioner has filed for dissolution of marriage. It may be accompanied with a Notice to Appear, and it informs the respondent that if he or she takes no action within 60 days, the court can grant dissolution without him or her. It also designates the manner of service.
- Verified Waiver of Final Hearing: Form PS-31152-2. When the couple agrees to all the terms and conditions of their dissolution, the Verified Waiver of Final Hearing dispenses with a final hearing on the dissolution.
- Settlement Agreement and Decree of Dissolution of Marriage: Form PS-31152-9. The couple signs the Settlement Agreement when they agree on all the terms and conditions of their dissolution. The Decree of Dissolution, signed by the judge, ends the marriage.
- Schedule for Visitation of Minor Children: This schedule spells out the custody regime and establishes the visitation routine for the custodial and noncustodial parent.
- Child Support Worksheet: The Child Support Obligation Worksheet (CSOW) calculates child support paid by each parent. The CSOW is normally filed with the Parenting Time Credit Worksheet (PTCW) and may be filed with the Post-Secondary Education Worksheet at the Provisional Hearing. Couples with children file these forms whether or not they have come to an agreement on the terms and conditions of their dissolution.
- Child Support Schedules: These schedules show the amount of child support based on income of the parents and the number of children.
- Declaration Under the Uniform Child Custody Jurisdiction Act: The UCCJEA declaration certifies that the parties comply with the UCCJEA and establishes the place of residence of minor children involved in the action.
